A assessment and action record for adults and their support team to use together, in an effort to identify people’s strengths and to plan support services that are based on person’s needs, choices and expectations. Commitments will then be made toward achieving those important outcomes. OAC places the individual at the center of all new support planning and takes on a new approach, a definite «Person Centered Planning”.
  • Social Role Valorization (SRV) is the backbone of this support. The major goal of SRV is to create or support socially valued roles for people in their society, because if a person holds valued social roles, he is highly likely to receive from society these good things in life that are available to him.
  • OAC places the individual at the centre of the entire planning process. The person for whom the planning is occurring is always present and the primary participant.
  • OAC involves the natural support network – family, friends and neighbors – as well as the organized support team – volunteers, professionals and facilitators. The staff members play a unique role in this process as they provide facilitation to the OUTCOMES.
  • OAC is ongoing and only ends when the individual decides it should end. The Intervention Plan is continually updated.

The aspects of the Regulation 299-10 are incorporated in the tool.
